Pitfalls
Recognize symptoms and steer against them
Agreer inflation
8 people are Agreer, everyone has veto, decision is blocked.
Agreer only for mandate-based veto right (legal, compliance, finance). Other stakeholders are Input.
Recommender and Decider in one person
Decider is also Recommender, method becomes self-approval.
Enforce separation. If impossible, decision is internal and does not need RAPID, but ADR.
Performer check skipped
Decision made, Performer team does not know it, implementation stalls.
Performer must confirm feasibility in writing in Phase 3. Without this check, no decision.
Pre-read ignored
Meeting becomes brainstorming because Agreer and Decider have not read the doc.
Recommender reminds 48 h before meeting. If someone is unprepared, postpone; do not hold meeting.
Decision without communication
Informed list is forgotten, employees learn later.
Communication plan as mandatory follow-up. Channel and timing in doc.
